Greetings to all, please help me have a link/URL of Visa on Arrival Application Form. I have been tirelessly searching for it, but all in vain - even the Thai embassy website isn't of any help. I am traveling from India to BKK from 20th April to 30th. Please help.

Thanks a ton Maestro, so I just need to duly fill this form, have the VOA fees ready and return tickets which I already have....would these 3 be all ? OR would I need more forms/checks prior to arriving.

The form asks for a passport size photograph (4x6 cm) . Nothing more needed, as far as I know. On the form, flight No. is your arriving flight. Keep the boarding pass, but it is highly unlikely that you will be asked for it.

Did you use this application? Was there any trouble? I am heading to Thailand this month end and I couldn't find the application anywhere but here, just wanted to make sure this application worked.
