I have three months before I request a one year extension for my Non-Immigrant O-A Visa. This is my first request having moved here last October. Just to be ahead of the game I asked my local branch at the SCB Bank in Lumpini if they were familiar with the requirement and if there would be no problem completing it for me when the time comes. They said yes and showed me the form, it was called " Statement of Income Request Form". Well to me this form title made me suspisious so I thought I would ask around on the Thai Visa Forum site. If anyone else here banks at SCB and has gone through the process and successfully used this form please let me know. I would hate to go to request the extension and something not be acceptable. If you were given another form also please let me know the name of it. This is my first post here and although I have read much of the archives I hope this question hasn't been asked a billion times. Thanks in advance for any tips or information.

Fist was need to know what you now have - is it a one year stay on a real non immigrant O-A visa issued by a Consulate? If it is do you have the multi entry version (which would allow another year stay with just a new entry)?

Believe you are planning to use 800k in bank account 3 months? For that bank passbook/copies and letter of account balance on day of application or previous few days will be required (so that letter title does sound a bit strange).

I obtained the O-A visa through the Thai Embassy in Washington DC before I moved and yes it is the multi entry version one year retirement style. Also you are correct that I plan to use the 800k that has been in the bank here since last October. I think you are more inline with the passbook copies and just a letter from the bank as you stated. The word income on the form they showed me made me think that something could be wrong and hence the post. Hopefully some SCB customer that has gone thought this can chime in. If the visa has not expired (and moving here last October it would seem unlikely) you can get a new one year stay at any time before that expiration date with a simple exit and return (and perhaps a visit somewhere). That would give you another year before you need to worry about a new extension of stay in Thailand. If you really do not want to travel then yes you can extend during the last 30 days of current entry.
