If you leave the country before 90 days, your 90 days kicks in again from the day you return.

You have to report every 90 days (if you are in country) and you have to renew your extension once every year.

Actually you renew extension every year and if you do not leave Thailand report address every 90 days but that can be done by someone for you or at most places by mail so actual visit is once a year if you want.
