What's Thai Form Mean Exactly And Where Need To Apply ?

I think I have to see this document in this forum, it's something it allow to leave the child out of Thailand,

but I don't know any more what's mean. It's seems impossible to take the text from PDF to translate into Google.

Can you explain what's mean exactly this documents ?

and which admnistration need to apply ? usually in living in Bangkok.. or province.

Don't now the name of the form, but another membr posted about it and he got it at the local amphur, where the other parent has to sign.

My limited Tha tells me it asks for the name of the parent giving permission, the adress, the names of the children it concerns and the name of the adult traveling with the child(ren).

Then it is signed by two persons, probably both parents.

For a much better translation, ask in the Thai language section.

Its a permission slip for children under the age of 20 to go holiday with someone thats over 20, which is named on the slip.

Sorry but wouldnt your wife or whoever be able to help you with this form?

I mean if you cant understand it then how will you be filling it out and know what to do with it?

Something doesnt seem quite right about this to me.

My Thai Girlfriend are..very stupid ( sorry ) about paper work in Thailand.. I say honestly..

I had help to her to make a Thai passport for her.. and make a new birth certificate.. and ask always on this forum most time.

I'm foreigner, but she does'nt know about to make paperwork in Thailand, as well she have study about Human Sciences( something like that) etc in University..

Hmm, well its a permission slip to take a child/minor out of Thailand.

Im not helping anymore than that sorry.

So, if have the form writted, it need to be registred by the Amphur to be valid, need some stamp, or don't need ?
