Does anyone have an English version or some kind of sample of the tax form 91 to file income tax 2012?

I got the form in the mail but it's in Thai. Would like to (try to) fill it in myself.

I do't have any income in Thailand except some bank interest ... paid about 8000Baht Tax on the interest. I'm taxed in the UK on my pension and on rental income. If I claim this tax back will I be opening a can of worms and have to declare my UK income and they will try and tax it ... I bring money in as and when. thanks.
