Re entry permit form

I have had one retirement extension but don't remember from last year so unfortunately have to bother "The Thai visa specialists" also this year. First the re-entry form. Near the bottom "My previous visa for Thailand is in the category of" Shall I here write the visa details I got last years extension on or when I got the extension ( extension is not recognized as visa I believe!) and further " I arrived in Thailand on" is that last time I arrived or when I arrived after getting the non immigrant visa the extension is based on some two years ago?

The visa category is the visa you obtained the extension on last time. The "I arrived in Thailand on" is your last entry to the country, ie the entry detailed on the entry card you must still have in your passport.

I allow me one more, and this time regarding the main "Extension of stay" where its also a question of " kind of visa" and arrival date" date. Is this referring to the visa I obtained the extension on and also my last entry to the country?
