You can apply for the extension of stay during the 30 days of your permit to stay.

Proof of 800k in the bank for 60 days proven by letter from bank plus bank book (copy also) to prove the 60 days.

Or 65k income by a letter from your embassy.

Or a combination of both totaling 800k baht.

Copies of passport photo page, visa and entry/permitted to stay stamp. Copy of TM6 departure card.

Proof of address.

Completed TM7 application form with attached 4cm X 6cm photo.

If you really have an O-A visa and it is multi entry you are aware any entry during validity gets a new one year stay? If it was single entry or expired do as above (although I would personally suggest money in bank three months if possible - technically the O-A visa is a pre-approved extension of stay so you might find someone unwilling to accept your new application as a first extension - have not seen any reports of this so do not believe it will happen - just my normal cautious nature).
