Obtained in March 2014. Cost was 40 USD per entry, so $80 total, must pay in USD, next day service, no lineups at any time, office close to Old Quarter area. Showed air tickets in & out. Reports from last year indicated they allow triple entry as well.

Was that tickets out of Thailand for each entry?

Personally i submitted 4 tickets, 2 in and 2 out of LOS, in accord with previous reports of last year:

"They were more than willing to give me a Triple-Entry Tourist Visa but I had to show proof of flying in and out of Thailand for each Visa"

"Ah, I remember someone else talking about the in and out if you wanted multiple entries. Crazy ! Nice little place they have there and as you say, no fuss and no muss."

I didn't. Even with many previous visits & tourist visas to LOS, including a couple in my present passport. The lady just asked a question or two, including why don't i get a retirement visa (or whatever you call it) to LOS. Told her i'm retired, don't work, but enjoy spending time in other places as well as Siam, which was supported by the stamps in my PP indicating 2 or 3 month departures from the Kingdom after every trip there. Though the actual reason i leave regards tax residency status & income tax.
