You will need the original and a copy of her birth certificate in Thai or English and her passport photo page.

Copy of your passport photo page and extension of stay stamp.

To get the visa company documents will not be needed. You just need proof of relationship and your extension of stay.

Dependent upon which immigration office you need to use you might need something from the company for her extension as your dependent.
