Nope.

Proof of address should be enough

Depending on your immigration office, you MAY need photocopies of Passport, current extension stamp, original visa and arrival/exit card along with the proof of address. Sometimes these are wanted at Jomtien, for example, and  sometimes handed back to you "for next time."
