If talking about inperson 90 day reports do not normally require photos - nor copies of anything at most places - but specific locations do sometimes have additional requirements beyond the normal TM.47

At most offices you only need your passport and the TM47 form.

You need to check to with your immigration office to see what they require.

I have never hear of a photo being needed anywhere. Just passport copies and TM6 departure card when needed.

I made a packet of copies that includes just about everything I could think of, passport, house book, drivers license, income docs.... I take it with me whenever I do anything official along with passport.. It works fo me, so when someone throws me a curve ball I have half a chance of having the paperwork they are asking for and don't have to make a return trip.
