TM 7 fill-in question for Samui

So I want to fill in the TM 7 (visa extension) form on my computer BEFORE I go to immigration. Just looks more professional and might smooth up the process, or I just want to flatter myself, who knows... anyway:

Can someone please tell me about the correct address part on the second page? I stay in Lamai (though apartment address says something about T. Maret). What is the correct input for:

1. Tambon/ Khwaeng

2. Amphoe/ Khet

3. Changwat

Finally, the first entry on the first page (immigration office): Do I write "Nathon, Koh Samui", or just "Koh Samui"?
