Can this be done after arrival, in country, or do I have to leave and apply outside the country.

It is possible but it can only be done at Bangkok or 3 other designated offices dependent upon the region of the country you are in.

The documentation requirements are greater than those to get one at a nearby embassy or consulate. Most people have to go out to get one instead of doing it at immigration.
