90 day report

When I do my first 90 day report, what should I bring to the Immigration office in Bangkok.

- passport

- departure card

- 90 day notification slip (if notification done before)

So it's only my passport that I need to bring there? All the copies I can make at the Immigration office in Bangkok, If I go 15 days before, will my next visit be 90 days from that day? Or 90 days from the original date ?
