Does anybody know where the REGISTRATION FORM FOR THE OWNER OF RESIDENCE can be downloaded?

Apparently, this is required by some immigration offices when applying for extension based on retirement.

Here: http://www.immigration.go.th/nov2004/download/tm30.doc This is the one for home-owners, although Phuket seems to have its own form) There is no other form.

http://www.immigration.go.th/nov2004/download/tm28.doc This is the one for foreigners reporting their address.

I am in Phuket and I think you are correct; Phuket has its own form. I guess a trip to the office is necessary?
