The cost is 1900 Bht You will need copies of your passport data page, entry stamp, departure card and one passport photo There are now two Immigration offices in Chiang Mai, the OP will have to go to the one at the Promenada shopping mall. There is a shuttle bus service between the two offices, so if it's easier for him to get to the old Immigration office close to the airport, he can take the shuttle bus from there.
