Hi has anyone filled one of these out before? If so where it says "Applicant’s Father Name" underneath it says "Nationality, Nationality at Birth".....etc etc.... Do I have to put my fathers or me?

If applying to London by post allow 7-10 days days for the return of your passport.

Many thanks that's a great help. Sorry to ask another question, but I have been trying to get through to the Thai Embassy in London but never can. I am wanting to get a triple entry tourist visa, and I want to stay between 6 to 9 months, I am staying at meditation retreats and monasteries, do I also need to fill the part that says "Evidence Substantiating your purpose of visit FOR NON-IMMIGRANT OR LONG STAY ONLY?" I have acceptance letters from the retreat centres and monasteries.

You are applying for a tourist visa therefore the purpose of your visit is "Tourism"

Ignore questions about Long Stay etc which are concerned with a very different type of visa.

Do not send any letters of invitation to the Embassy. Doing so will complicate and confuse your application.
