OP: just a thought, but you mention that you do "heavy strength training". I don't know how heavy is heavy, but there is something called "Over training syndrome" which causes chronic fatigue. It is mainly limited to competitive athletes because it occurs only with extremely large and frequent exertion combined with insufficient rest.

I don't know if what you are doing is extensive enough to make this possible. But in any case you should adjust your level of physical exertion to what seems to leave you most energized in the intervals in between. As you have a history of cocaine abuse you are likely one of these people with a tendency to get "hooked" on surges of adrenaline. They aren't good for you and over time will contribute to chronic fatigue.

I'm not a doctor but this could be environmental factors. Try checking yourself into a good temple away from the cities for a week. Clear your mind, your energy and your body.

What is fasting glucose level and score on and A1C test ? Pre diabetic that most Dr.'s don't even bring attention too plays big role is energy levels . Also full thyroid panel massive amount of population has low thyroid and again Dr.'s miss until it get bad and then just treat symptoms much of the time . Get that full thyroid, fasting glucose level and A1c test and really investigate the numbers .
