First is normal physical exam and symptoms from Mayo clinic Google with additional tests as indicated. ABI is simple few minute test and not very expensive (have had done in the past). Ultrasound and angiography (various types) are also listed and would be more expensive - so really would need to know what is required for posters to make a good estimate.

Sorry I do not recall cost of ABI and was some years ago but do recall did not consider it expensive. If nothing found that may be all that is required - or maybe some other factor will be found for symptoms and not test required.

Is it triggered by walking./exercise and relieved by rest?

If so, should see a vascular specialist.

ABI as mentioned is inexpensive and easily done. Depending on results of physical exam might or might nto also need doppler. Where in Thailand do you live?

It's constant and rest does'nt alleviate the problem, foot massage is good but only lasts about and hour or so.

Always had a problem with aching feet (have orthotics), but now the soles are warm.

legs don't seem to ache while exercising.

Doesn't bother you during exercise, and not relieved by rest = not PAD. If it were PAD exercise would provoke the pain and rest would relieve it.

As to what it is: sounds possibly like some type of neuropathy. Are you diabetic?

Not that i know of Sheryl , but Diabetes check up will be the next thing to do.

Last check up was early 2014 and was ok then.

If you did not have diabetes a year ago, unlikely to have diabetic neuropathy now (though should of course have yearly checks of fasting blood sugar).

I am puzzled re your condition and unsure what type of doctor to suggest you consult. Can you describe the "ache" a little more, what does it feel like? And what exactly do you mean when you say the soles of your feet are "warm"? A burning sensation?

Lastly what medications are you taking regularly?

The ache is like sore muscles after exercise. Few times a day get a shooting pain in calf muscles

The feet i woudn't say burning but feel hot(not as in touch)

If i elevate my legs the feet soothe a bit

No medication , just been taking Centrum vitamins last six or so weeks.

Best guess I can make is that it is either related to your orthotics/feet problem and that perhaps the orthotics aren't quite right for you and causing strain on some muscle groups or (2) some type of neuropathy, though the description of the pain doesn't really match. A third possibility and one consistent with both the aching feeling and the relief obtained by elevating your legs would be varicosities but those should be visible to you. If by any chance you do have visible varicose veins that you didn't mention then that would likely be the culprit.

Less likely possibilities include early stages of various neuromuscular diseases and a spinal problem.

Since there are many possible causes it is a little difficult to proceed. But as the orthotics issues seem most probably (and could certainly lead to aching calves) I suggest you start with this doctor:

He also always used to be at Bangkok Hospital Foot Clinic but they have revamped their website and it is a mess and I can't get it to show his name. Try their call center 1719

If this does not work will have to move on to other specialists, but start with this as with symptoms in the foot and then aching calves, in someone using orthotics, seems like the most likely bet.

As mentioned it cannot be PAD if it isn't made worse by exertion/relieved by rest.
