I have no direct knowledge of the precise stage of membrane development when surgery is definitively advised. It was traditional, yes. But a 2004 study indicated benefit to doing it earlier, see http://www.ncbi.nlm.nih.gov/pmc/articles/PMC1280091/ Of course, most epiretinal membranes do not require surgery at all.
