Visa Extension - change of address

When I moved to Th I had to use my company address for the visa extension, I am now going to apply for my own visa extension with my home adress, do I need to fill in any extra documentation for the move of address? by the way I did my first 90day reporting on Friday using my new address for the first time and nothing was said.

If you have already notified them of your change of address to your home address via the 90 day report I doubt they will need anything else for the new extension. However, they may want proof of address for the new extension and or a TM30 completed by the owner/house-master of the home address.
