Letter from previous employer for new NON-B One year extension., Is there any sample available?

I am leaving my job, so can i make it now for future use?

Sounds dodgy to me.

My employer is very good with me, but employer not aware about legal formalities, i have to help for documents.

If you are leaving the job you cannot "make" an extension and if you already have one it becomes/became invalid on the day you ceased work.

I already canceled my non-b visa, i am in Thailand now with tourist visa.

WP already canceled by employer, i requested to employer for my social security documents.

is there any form or sample from government for letter of job leaving?

so i can give it to employer and they will make the document for me.
