Did go to Phuket Immigration, to get my

90 day visa extension

but now they have a new form to fill out, and a copy of the first page in my passport, was needed. The form is a "multi"-form for different use. You can see it here.

Yet another incarnation of the infamous information form.

Take a magnifying glass and a very fine pen with you

 

The photo frame labeled with "NO picture" is "inventive"

(at least an answer to the question "is a photo required")

 

"90 day visa extension" ? There is no such thing.

Yes that is correct: 90 days report!

On the question from me: Why a new form?

The offiser replayed: I got a new computer.

And indeed he had two computers now.

 

Just click on the form and it gets easier to read it.

That would be a surprise if they enter the data to an electronic database
