It is clearly spelled out with the form available for download and filling out (you sign in presence of Consular Officer). Download and fill out form from this page https://th.usembassy.gov/u-s-citizen-services/local-resources-of-u-s-citizens/notaries-public/income-affidavit/ Make appointment or attend outreach to have form signed https://th.usembassy.gov/u-s-citizen-services/acsappointment/
ubonjoe
They don't verify your income. As said you complete the income affidavit and swear an oath in front of a consular officer it is true and correct.
keeniau96
Remember it is a sworn affidavit which is no trivial thing, lying under oath is perjury with 5-year jail sentence in USA.

Are you aware if it is the same for Canadians, where they do not actually verify anything?

kokesaat
The American Citizen Services provides outreach services around the country. Might save you a trip to Bangkok. https://th.usembassy.gov/u-s-citizen-services/consular-outreach/
