Hi I am about to start HIV treatment in the next few weeks.   Cd4 289 v load 3000; 21% cd4I'm just wondering what generic combination therapy other expats are on and cost. Atripla has just been downgraded. I just need to go to my specialists with as much info about drug therapy as possible. Thank you.

The change in recommendation re Atripla was in the US. HIV drug sensitivity/resistance patterns do vary by part of the world.  So it matters where you were initially infected - US, Thailand or other.

 

The Thai HIV treatment guidelines have not been revised since 2010 and still consists of:

AZT+ 3TC  (lamivudine) + EFV or (if cannot tolerate EFV) LPV/r

 

OR

 

TDF  (tenofovir)  + 3TC + NVP or (if cannot tolerate EFV) LPV/r

 

Atripla is a combination of EFV,  TDF and emtricitabine (FTC).  So as you can see, has not been being used as first line treatment in Thai patients though it is available both singly and in combo form.

 

I think the most important thing for you is to get expert opinion from an HIV specialist with regard to the strain of HIV you likely have based on where you contacted it.
