NonImmO Retirement - Docs required in Thai - translation help?

Hi,

 

This will be my first Non Imm O Retirement extension outside my home city of Bangkok due to family issues and when I went to the upcountry Imm Office they gave me this list of documents required to apply for an extension there instead of Bangkok.

 

** Unfortunately its all in Thai which seems rather odd for foreigners! I can understand most of it, but can someone clearly explain what is item#2 (apparently crossed out and not needed?) and what are items  #8, 9 & 12 ?

 

Appreciate any feedback, I understand the other items so no need explain them thanks.

8: สตม.2

That is a form where you undersign that you have understood the terms of your stay and will inform the office about any changes.

It's a bilingual form which I have also seen in the past.

Nothing but a signature, a formal thing.

My office gives it to me as a 2 sided copy. The other side is an acknowledgement of the overstay rules.

They will give you all the forms to complete when you do the application.

that sounds like "other documents to the discretion of the officer".

A clause that you find in official immigration documents also.

For a simple retirement extension there should normally not be any "special" documents needed.

Except for some offices known for their notorious harassment (like health certificate, photos and map of house location even for retirement extension etc.).

Thanks so much to KhunBENQ and ubonjoe for answers. That is much clearer for me. Even my wife didn't understand #8 when I showed her at home, but I remember a similar form last year when I did it in Bangkok! Anyway I think #12 won't be much as they are quite helpful otherwise at this other office which is in Nakhon Sithammarat.

I have seen this same document with a page 2 attached to it called "The Acknowledgement of Penalties for Visa Overstay" to be completed also? Is there any where I can download this page 2, not sure if its part of สตม.2 or just another form that had been shown together with it before?

I have not seen a download for the acknowledgement of the overstay rules.

My office gives me one sheet of paper with the acknowledgements on each side of it to fill out.
