Toilets south of the 60th parallel and north of Mexico are free, plentiful, and cleaner than many Thai public toilets. You can walk 300 meters in a fancy Thai mall before finding a toilet.
Mai bpen rai makes a lot of sense, often. At other times it's absurd.
You don't need to ride a 1000 cc bike or drive a big car with 400 cubic inch V8.

I have always only used Chiang Mai Ram as an expensive last alternative to Sripat. I wonder if CMU Medical School professsors/oncologists practice at Sripat/Maharaj (almost surely). A Thai ajarn I worked with was treated for breast cancer; I saw her getting a checkup later at Maharaj.

Breast feeding was not widespread in Texas, but okay. In southern Mexico, especially among Mayas, it was nearly universal, for several health and economic reasons. In northern Thailand, very rarely seen.

I lived in US over 40 years. Maybe got 2 flu shots. Maybe a virus flew down the chimney flue on a flea. Don't know if I ever got flued, Went for vaccination update laast month and opted only for inoculation against (Japanese) encephalitis since Mom got that in 1948 (but she died from penicillin allergy).
