The reference to rudeness was not regarding you. I was talking about how Thais respond to people in wheelchairs. Which is the thread topic. As Moderator it is my role to point out when peopel are ignoring a poster's stated wishes in regard to information. The OP has said, repeatedly, that he does not want to discuss his diagnosis or any treatent options. (ANY - regardless of type). and he has chosen not to provide information on his diagnosis, which is his right. Please respect this and let it drop unless you have something to contribute on the subject of wheelchair use.

This is quite specialized. OP needs not just any orthopedic surgeon but one with considerable expertise in treating rotator cuff repair. None in Hua Hin. Hua Hin is a just a town, not a provincial capital or municipality, and its local government hospital is just a community hospital. Even travelling to the the provincial capital of Prachuab Kiri Khan, it is just a genera hospital, no region level facilities near by. The private hospitals in Hua Hin are really small with limited range of doctors. And very costly, more so private hospitals in Bangkok, as there is no economy of scale. It is quite common for people in Hua Hin to need to come up to Bangkok for anyhting other than very routine care. Sometimes the local Bangkok Hospital affiliate will arrange to bring down doctors or equipment for a specific patient but when they do that it costs vastly more than same thing would at their hospitals in Bangkok. On the positive side, no need to go into downtown Bangkok, there is a wealth of medical expertise on the Thonburi side of the river and not far from the southern bus terminal. -
Unless by visa he actually means permission of stay. Not unusual for people to refer to extensions of stay as visas. OP needs to clarify.

To be clear: Is this a newly issued visa, not yet used, or a multi-entry visa, with visa expiration 23 April? or Have you already been in Thailand on this visa, and 23 April is the expiration of your current permission of stay/last extension of stay? (In which case - did you get a re-entry permit before you left?)

iHerb seems to have worked out well how to label the packages and also what limit to ship (you will get an error message if you try to order more at a time, in value, than they can be delivered without issue. Haven't had any customs issue in years using them How long to arrive will depend on the item and where you live. I usually get in a few weeks.

Do not have this done in Hua Hin. Nowhere near the same level of expertise as in Bangkok and will also cost more for what may be worse results. You will need to come up to Bangkok, many excellent shoulder specialists there. Before I suggest one please indicate: - where is more convenient for you, Thonburi area (closer drive from Hua Hin) or central Bangkok? - can you afford private hospital? (350 - 400k baht roughly) -

That picture does not seem to show a mass, just redness and bruising. But your post implied something hard and raised. Certainly this does nto look like a cyst or abscess. Do you actually feel a hard lump under the sin? If not then it may just be irritation, perhaps with mild secondary fungal infection as easily occurs whenever there is irritation in that region. it were me, I'd apply a cream containing both steroid and antifungal, something like Myda-B (available everywhere) and see if that does the trick. If not then see a dermatologist. But likely it will. -
Without any information on the cause/diagnosis it is pointless to give advice about treatment, and OP has made it clear he is nto interested in discussing this but only in discussing the ins and outs of wheelchair use in Thailand. OP I have had experience having long term visitors who were wheelchair bound. It is not as easy as it is in the west, especially in terms of trying to go anywhere on a sidewalk (sic) but it is certainly possible and with time one learns the tricks. Most places have handicapped parking places. Sometimes coned off, you have to lean on the horn and wait for someone to remove or else driver get out and do it. Some hotels and resorts are handicap friendly and most booking platforms let you select that as a filter. Some malls/supermarkets have motorized carts for use by people who are w/c bound. As to people's reactions, all over the map ranging from very kind and considerate to rude. Take it as a sign of what kind of person they are. You will on the whole encounter more kindness than unkindness but it will run the gamut.

Could be cyst, an absecess, or a lipoma. And if a cyst could be a complicated type (pilonidal) You need to see a doctor. -

There are specific tests that can be done (outpatient) if a motility problem is suspected. "Motility Center" is a bit of an exaggeration. It is the regular GI center. With capacity for some motility tests. Actually gastric pain is often felt in ghe chest. Can even mimic heart attack.

With chest pain the main differentials are: 1. Cardiac cause (angina, heart attack). This is an emergency but would usually be described as a crushing pain/pressure and more often on the left. Can't exclude this but your description does not raise immediate alarm bells. 2. GERD (reflux). Your mention of burning and of it being triggered by lying flat or bending over is suggestive of GERD though there would usually also be some throat discomfort/burning. Reflux is common in older people and people of any age if overweight. Also often linked to caffeine intake. 3. Gastritis. Consistent with location and description of the pain but not so much with being worse lying down or leaning over. Gastritis and GERD can coexist 4. Muscle spasm or injury - in that location this would be usually be what is called costochondritis though that pain is usually described as sharp. Most often there would be a history of injury or strain before the onset. Strain could include things that required upward movements of the arm or twisting of the torso. Rowing, weight lifting, various sports can cause it. So can severe coughing. Any history of these? 5. Least likely would be an issue with the lungs. Can't rule it out but I would start with other possibilities first. If it were me-- and assuming there was no strain or injury that preceded this -- I'd start by seeing if antacids and PPIs (omeprazole etc) give relief. In the case of antacids you'd be looking for temporary improvement whereas PPIs would give longer relief or prevent the pain altogether. I would also avoid lying or bending over for 2 hours after eating. If these things work then GERD and/or gastritis is likely cause but to be sure would need endoscopy. Also should exclude h. Pylori, common cause of gastrutis. For this would see GI specialist. If you do have a history of injury or strain that might have inflamed chest muscles, rest and NSAIDs usually do the trick but be very careful re NSAIDs as if the problem is gastrits it will make if much worse. Also there are contraindications to NSAIDs that should be researched before taking. If the pain becomes severe and if there is any radiation to left arm or shoulder these are signs of posdinle heart attack and shoukd go at once to hospital.

Can you please describe the pain more clearly: 1. Where in the chest do you feel it exactly? You say middle but how high up,? Does it extend to base of the throat? 2. What does the pain feel like? Burning? Cramping? Achy? Pressure etc? 3. What if anything seems to releive it?

US prescriptions have absolutely no validity in Thailand. Nor do prescriptions from any other country. Same is true in most countries. I doubt you could walk into a US pharmacy and fill a prescription issued by a doctor in another country. While many drugs that are prescription only in western countries are sold over the counter here, the reverse is also sometimes the case. Every country has its own rules about this. In Thailand, as in most countries, Tamoxifen definitely is not an over the counter drug, it is in a special controlled category. Pharmacies do not carry it. You also need to understand that there is no real "prescription" system in Thailand, no prescription pads etc. Drugs that are by prescription only usually have to be gotten from a hospital or clinic pharmacy and these disburse only drugs ordered by doctors on their staff. There is no system for "refill" of such drugs, have to see doctor again. This is why her doctor in Bangkok could not do anything to help her get it in Samui. This was not a "refusal" it was people following the rules and system that is in place in Thailand. -
Exactly. The key thing about a high power is not what it is (which will be understood very differently by different people - and by definition can't really be known in the intellectual sense) but what it is not. It is not self will. An excessive attempt to solve all problems through ego-centered self will (even when that is clearly not working) is typical of people with addiction issues. One has to do the leg work, of course, but a key part of all 12 step programs -- and one that works -- is learning to moderate excessive self will and accept help from outside oneself , be it from other people, the group, the program, nature or whatever higher power one can accept. It is a complete red herring -- and very counterproductive -- to get caught up in ideological beliefs about higher powers. People do this for various reasons. Some because they are unwilling to relax their pride/self will even an iota; some because they have had prior negative experiences with organized religions and mistakenly think this would be something of that sort; and some because they are just looking for excuses nto to try a 12 step program.
