Take your old a new passport to where you obtained the vaccine certificate, I'm sure they will update a new certificate with a new PP number, MOPH would then be updated at the same time I think, thats my plan anyhow.......( carry old PP with new PP for awhile as well ).

You could whip over to your nearest Health Ministry Department with your CV vaccine certificate and get the International yellow CV vaccine passport/booklet. I think online booking appointment is necessary for Bangkok, not sure if useful arriving state side but handy for returning here.

My thoughts as well, Im double jabbed with AZ, a JJ would seem to be the correct 3rd choice being a viral vector. I bought a Moderna 2 days ago but could possibly give it away - unsure ( as many are ) what to do 5-6 months down the road if anything at all.........

I had eye (ophthalmology) appointments recently bkk hospital, Dr consultation was a flat 1200 baht each time plus the small add ons prior to - BP/TEMP. etc. possibly similar to orthapedics.......

18 or more months now of survival/desperation, ramp up vaccinations, buckle down 3 more months get the local vaccinated percentage way up........ get fixed up from within....

JJ and AZ are viral vector, 6 months after the second AZ if available or even if recommended at the time interval per say, should JJ be the booster if double vaxed with AZ ? or AZ again for that matter......
