I understand that supporting documentation for TM 30 is dependent on the Immigration Office involved.  

In this case it is the Khon Kaen Immigration Office.

I am moving to KK next month to live with my thai gf in her house which had recently been built. 

In addition to the TM 30 and a copy of my passport page, what else does KK require.

Grateful for relevant info from any one using the KK Immigration Office.

I cannot speak to KK specifically, but I would bring the GF (who is also your "landlord") - plus her house-book - plus official-docs showing she is the owner of the property - plus some sort of rental contract which allows you to live there (since you are not married). 

Signed-copies of your "rental contract," her ID-Card, and her Housebook will likely be needed. 

I doubt they would ask for more than that list.

Immigration will accept a short written statement from your girlfriend that you are living with her with a copy of her house book and ID card attached as proof of your residence.

Some offices want a TM28 form done by you for a change of address as well as the TM30 form from your girlfriend.
