Health form for extension based on retirement

Some questions about the health form that is (is not?) required for extension of visa based on retirement:

 

1. Can someone provide a blank form?

2. Can anyone recommend a clinic in Bangkok to get it with a minimum of fuzz?

3. Anyone with recent experience in extending a NON-O based on retirement - did they ask for the health form?

One or two rogue immigration offices were said to be requiring the health certificate for the retirement extension but hopefully they have been pulled back into line by now.

I wish.........Amnat Chareon and Roi Et were still requesting them for Retirement extensions as of last year.

Stupid as your never examined and the general health certificate only covers leprosy, tuberculosis and elephantiasis.

It also looks like they are still mandatory at Samui, according to post #107 in this thread:-
