Can someone direct me to a safe site where I can download the form (TM8, I think) for multiple reentry permit?  The links on official sites I've found seem to be broken and the others I've found are on sites that appear a bit sketchy.

Ubonjoe - on the fillable TM7 I can fill in blanks but when you go to next

field the prior field disappears.  Is there something I’m doing incorrectly?

It works good for me using Adobe Acrobat Reader DC.
