I have a non o multi entry visa based on marriage from savannakhet.

 

I recently renewed my passport. But the visa is in the old passport.

 

Do I take both passport with me when I exit Thailand or do I need to visit Thai immigration to transfer over the visa to my new passport

When you exit Thailand the first time with your new passport, present your old one as well.  They will transfer the info from your last permitted-stay to the new passport.

You will only need the old-passport to exit the first time.  But, for as long as you wish to use your existing visa, you will need to show both passports when ENTERING Thailand.  The "Visa" cannot be transferred - only your permitted-stay, so the IO will reference the Visa in the old passport.
