My brother he's 48 y.o and he want to come in Thailand for 5 months what would be is best option?

It would be is first time in Thailand.

I would suggest a single entry tourist visa (to cover the first 2-3 months, depending on whether the 60 day permission to stay on entry is extended, as allowed, for a further 30 days). I would follow this by a trip to somewhere like Kuala Lumpur or Ho Chi Minh City for another tourist visa. Some will suggest a multiple entry tourist visa, but I would assume someone on his first trip to Thailand would be interested in a side trip to an interesting neighbouring country. Applying for another tourist visa is not going to be too much of a hassle. If, however, a lot of travelling around the region is contemplated, applying for a multiple entry tourist visa would be best.

A multiple entry tourist visa visa would be a good option. It allows unlimited 60 day entries for 6 months from the date of issue.

Or a single entry tourist visa that would allow one 60 day entry that can be extended for 30 days and then another one from a nearby embassy or consulate. Or after the tourist visa a border hop for a 30 day visa exempt entry that can be extended for 30 days.

There is no visa he could get that would allow him to stay for 5 months without leaving the country at least once.

There are no 5-month visa. A single Entry Tourist Visa is easy to obtain compared to an METV and requires less paperwork. He can use it for 3 months (with one extension inside). After that go to Penang and get another SETV. This will take him to 5-months.

There are possible ways, but they would not make sense. For instance, he could register with a Thai language school, arrive on a Non Ed visa (90 day stay) and get an extension of permission to stay from that. It would be complicated, expensive, and surely inappropriate for a casual visitor.

He could apply for Thailand Elite and stay 5 months without leaving Thailand.

No, but his situation is straightforward and shouldn't cause immigration problems. Get a home country SETV first (2 months + 1 month extension), then fly somewhere regionally for a short visit and apply for another. All the regional destinations are great to visit, so it can be looked on as an interesting and enjoyable break during his trip. His flight out at 5 months should satisfy embassies requiring evidence of onward travel. 

Check what the Thai Consulate in the USA wants to see to apply for the SETV - may require buying a ticket to KL or similar within 60 or 90 days of the ticket entering Thailand.  This can be used to travel for the 2nd SETV, or just a "throw away" ticket.

Another question in Cebu,Philippines, there a Thai consulate is it a good place for a Canadian to apply for another tourist Visa,or it's better he goes to Manila considering he will have been in Thailand almost 90 days and about 2 weeks in the Philippines?

The last report I read about a non Filipino applying was "They would not do it."  He could always try, and see if that has changed.  Otherwise, Manila is the only chance.
