I was given the attached form by the immigration department as one of the documents that needs to be submitted for renewal of visa extension on the basis of retirement. Has anyone seen this form before? I have done an online search for it on the internet to try and get some more information about it and I can't seem to find any reference of it anywhere.

 

I am also not sure I understand the exact purpose of the document. It appears someone else has to partially fill in the form for you on your behalf with some of their own personal details.

 

Does this mean you have to get someone else to certify and sign on this form before you can go to renew your visa extension on the basis of retirement? And if you don't have someone willing to sign the form for you, then will you be prevented from renewing your visa extension on the basis of retirement?

Appears to be some form of guarantor form for someone applying for an Extension. Never seen it before and notice it does not have a TM number.

My experience. Last month when at CW I was handed this form whilst waiting for renewal(retirement). When interviewed the officer asked me to just to fill in the essential name and address etc details.and sign it. At the counters for the 1 year renewal I notice workers were there who did need to fill in the whole form as they go to the same section as retirees.

I am not even sure this form needs to be submitted by retirees.

So for retirement my understanding is you can leave the guarantor part blank and hence no counter signor needed for u OP.

Hi Jim, 

As Jacko45k has indicated, there is no TM number. This makes me think it is a document developed by your local Immigration office. I have completed 3 Extensions for Retirement in Udon Thani and have never seen this form.

 

Which Immigration office are you using?

i was given the same form last year at CW.   IO had me sign it, wouldn't explain what it was for.  i had mentioned it in a write-up post about that day's CW experience -- at that time i guessed it "might" be for a landlord to verify.

 

on second thought, i think it might be an "anti-corruption" form dreamed up by CW, where the IO has to certify that you were present for the application.

now that's much better, was going to ask where or which office gave it to you because in Thailand each and every office has complete different documents/forms request, I do think that all IMO went to the same school but they all interpret the law as it fits their personal needs

In December at CW I was asked to complete the form. Filled in name and address but nothing else. IO did not say anything about it.

My head is going to explode with ALL the rigmarol and requirements for a first one year visa extension based on retirement.  This will follow a non immigrant O visa I believe  ??

There is so much to get your head around ......  why so complicated or am I overeacting in that it's not as complex as it seems ... ???   400k + 40k per month  ... or 800k for 6 months of the year ( 3 mths before apply and leave in for 3 months after apply )

Not sure how much of this will be relevant for you, but when I did my first one year visa extension on the basis of retirement I had started a thread, which had a lot of questions and answers in it as it progressed.
