Disabled / Sick, not able to travel to the Immigration office, extension of stay and re entry permit

Many here have probably read stories of people who were brought to immigration with an ambulance because they were not able to go their otherwise. I also remember that somebody here complained that the IO requested photos of him in his bed to accept such an application through another person.

I came across the official police order from the Immigration Bureau from about 2 years ago, which specifies the required documents in case the physical condition of a person does not allow him to go to the immigration office by himself and another person has to do this for him.

Submitting photos which show the illness is officially required under 3.4.
