Does any one have or know where I can find a translation of the Withholding tax form.

Asked my staff to do translate it but her english is not up to the language used on the form.

Any pointers much appreciated

Have an accountant help you fill it up once, and then memorize what numbers go where and on what part of the form.

You may write in English on the blanks provided for your name, addresss, etc.

Ive hunted around, and i think im just gonna end up paying Sunbelt to translate it for me. Its only 1 page so should cost 500 baht.
