I am not really clear what you need if the issue is a failed skin flap repair. That would usually need a plastic surgeon and not be an indication for Mohs.

 

Mohs is indicated  only if there is still cancer present, e.g. if the prior removal did not have clear margins. Even then, a good plastic surgeon may be able to do an excision without Mohs that gets clear margins without, or with minimal, scarring.

 

The issue with MoHs is that it is very time consuming and requires considerable work and availability from &b  coordination with the pathology dept. Hence while it sounds good in theory, even doctors trained in it (as Dr. Niyom is) don't do it often.

If the prior removal got clear margins and the issue is that the flap has failed, you may nto need Mohs but rather a flap revision by a plastic surgeon.

The bio of Dr. Gateorn Pongarnar at Sukhumvit Hospital says she trained in Mohs surgery in the US, but I don't know if she does it now.
