a reliable friend of mine has gotten the visa extention, with a printed out of the embassy page which stated that they do not issure letters. and normal application forms, a photo, a set copies of passport and visa extentions, and tm30 form. 

if you do not have the tm 30, you need ask you landlord to give you a set of filled 2-pages application forms and a screen printed out that shows that they did report you. you can use these 3 pages to get your new tm30 on site for free. (Do not ask me, I have no such experience, you landlords or hostel owners should know.)
