Which physician does the medical certificate for the special tourist visa application in the Bay Area (California)

I am trying to have physician complete the medical certificate for the special tourist visa application. I have been to or called a dozen of medical practices and nobody wants to complete and sign the certifice, mostly because in the United States, it's outstandingly rare to test for leprosy and elephantiasis.
