If I have the CoE and the PCR test can I just get the 30 day visa on arrival, can I extend this 30 day visa on arrival at immigration to get another 30 days and then change my return flight dates?

It's not a visa on arrival, it's a visa exempt entry.  Currently it's 45 days to compensate for the quarantine period.  And yes, you can extend by 30 days at the end of the 45.

As above, no visa on arrival; visa exempt entry if you are from one of the countries that qualifies for it.

True, and the practical implication is that you need to get into a different line when you hit immigration upon arrival.  The normal line... 

 

There's a separate line for visa on arrival.
