The best Dermatologist in Bangkok?

Hi,

Good afternoon!

I have the following symptoms and visited the Bangkok hospital twice. Prescribed the Urea 10% TA 0.1% and the Urea 10% TA 0.02% cream. I have applied it for more than one month. However, the symptoms have not solved.
   
1) Red spot on my back, image attached with arrow pointed, is very itchy for about 3 months.
2) I have hives for about 3 months.
3) My scrotum (a bit red, sometimes) and perianal area is itchy. I think they are allergic to swimming pool water, hence, 
I stop exercising in the swimming pool for more than 1 month. However, still itchy.  

Can anyone who has knowledge or experience about it kindly advise the best dermatologist in Bangkok?

If your problem is hives and they are not always present, just appear at times, you might do better with an allergy specialist than with a dermatologist as if it is hives it is nto really a dermatology issue.  You might try identifying the allergen yourself first. If it occurs mostly at night, consider possibly the detergent the sheets are washed in.

 

I cannot really tell from the pictures whether these are hives. Are they raised?

Yes, they are raised. 

 

My wife said she have been using same detergent. My living environment and diet are no change for years.

 

The hives occurs anytime from 7 pm to 12 noon (17 hours) during last 4 months. 

 

No occur after 12 noon, maybe due to I applied medicated oil to each of the different affected areas up to 3 times or took Levocitirizine.

I guess you have already tried an anti-histamine for the hives?

On a side note, my own experience has been that after 60 years eating copious amounts of bread, I suddenly for no logical reason developed a gluten sensitivity that in my case, the main symptoms were extreme generalised itching and some very localised rashes.....but no hives.

However, it took a long time to discover that gluten was the problem.....and a topical steroid relieved the rashes.

Total abstinence from gluten consumption drastically reduced the itching.....but not totally, I would still sometimes get a bout of itching.

And then after about 4 to 5 years the sensitivity has almost disappeared.

My brothers wife also suddenly developed a generalised itching and this too she discovered was due to a gluten sensitivity.

Anything is worth a try......... if you are consuming any gluten? ????

Apparently he is taking antihistamines and they are working, which rules out bedbugs and strongly suggests allergic hives.

 

OP unless the source of the allergy can be identified you are going to be stuck with antihistamines long term.

 

I suggest you see this doctor. Allergy testing may be necessary and while undergoing it you will need to stay off the antihistamines.

What do you mean by "generalised itching"?

It is great that you discover the source of itching. Can you kindly share how did you discover it and how the sensitivity has almost disappeared? Btw, congratulations!
I wish to know the cause of my chronic hives.

Mine is not localized and they are chronic spontaneous hives. I eat very little bread (maybe 10 slices per year maximum).

That is the purpose of the testing, yes.

the other way is trial and error.

 

Would help to keep a diary of what you eat relative to when the hives occur and also to see if they occur when away from home.

I take 4 supplements, 1 tablet each (Blackmores DHA fish oil, Magnesium Citrate 100mg, B12 and B6)
around 8PM daily for around 3 years and not on no other medications.

I have stopped taking them for around 2 months after 1st month of occurring hives, however, hives still occurred.    

Is allergy testing a blood test?

Not a doctor.

 

Could be some blood tests but mostly allergy testing involves subcutaneous injections of suspected allergans to see if they provoke a reaction. To do this you will have to stop the antihistamines for a while.  Doctors need to be able to see the response.

 

Do keep food diary and try to see if there ate any commanalities in when the hives appeat and what you ate (this too will require laying off the antihistamines). Akso ask your wife if there is anything new she started to use im cooking around the time this problem began.

 

And a short trip away from home to see if the symptoms occur or are limited to ypur home would be useful.

 

Aside from allergies the other thing that can cause hives is severe stress.
