Thai goverment Heath insurance  known as 30bhat scheme. how does it work ? does any body know a link/download in English of the policy documents ? i know this is for thai people mainly . but need information to see whats coverd for thai girlfriend ,from what I hear from her you have to be near dead for them to do and pay for anything , eg if cant poo/pee for weeks that won't do anything see says. but go in very ill and yellow near dead that may be covered.

There are no policy documents available.

 

It covers all medical and dental care, inpatient and outpatient. Including cost of medications. Exceptions are very few: some new drugs not yet on the MoH list, for example, and some expensive imported medical devices. And (at least until recently may have changed) hemodialysis was covered only in patients not able to do peritoneal dialysis. Even kidney transplants are covered, though criteria are stringent.

 

What your girlfriend told you has nothing to do with the payment scheme. It is true that government hospitals - regardless of what payment scheme one is under - only hospitalize people with strong indication, in order to conserve resources. And they often lack availabel beds.  Same as public hospitals anywhere.  Also true that they will often initially try simplest measures first to see if the problem goes away before going on to more thorough tests etc. They will always "do" something but initial approach to say, "can't poo for weeks" will be to dispense  a laxative and only if/when that doesn't work and patient keeps coming back, consider a referral to a specialist and scans or endoscopy.

 

They will definitely do more than something if someone has nto been able to pee for days let alone  weeks. Great exaggeration.

 

It sounds like she is just not happy with her local hospital. Nothing to do with the payment scheme.

 

As for not being happy with her local hospital - government hospitals do differ greatly in level and quality. Small community hospitals have limited capacities, often just a few doctors and all of them new graduates. Provincial hospitals are better, regional hospitals better still and teaccing hospitals best of all.

 

In addition, Thai expectations of what constitutes "good" care often is nto grounded in good medicine. For example, there is a strong belief that IVs are always best, followed by injections with oral medications the least best (and no medication, an unthinkable insult). This is of course not at all correct. But many people believe it and will complain that the hospital did nto help them because they did not get the expected IV etc.

okay it seems she not telling me the whole story about the 30baht scheme the problem She been having is about 10years ago she was very ill with not being able to poo , the doc said there is something wrong inside and said they would have to cut the bad bit out and join together if she cant poo she thinking money (no have). As you said she interpreted this as if cant poo she would have to have this done, But at her costs...   so she been taking Laxatives every day for the last 10 years,, sometime she still can’t poo for 1 week and stomach gets very bloated with air every day, then get pains till she has a poo but then not all come out.. This don’t seem good to me, she said if she went they would just give her more laxatives. The old Doc has problem gone now and would need a scan to even start again, would this be covered, I suppose if the doc ordered a scan that would be okay, But it’s getting past the laxatives hurdle

Yes, scan would be covered if ordered by a  doctor, but there may be a long wait list. Though colonoscopy would usually be first resort rather than scan.

 

Hopefully the hospital still has all her old records.


Which hospital is this? Where in Thailand is she?

 

The history as you relate it does not make sense, likely she misunderstood a great deal. Also, the situation 10 years ago may not be the same as now.  Apparently she did eventually poo 10 years ago avoiding the need for surgery. But has now become laxative dependent.

 

Was a colonoscopy or  scan done 10 years ago?

 

See if you can get the actual hospital records and post them here, with identifying info blacked out.
