Authorities have confirmed the death of a resident from anthrax in the Don Tan district of Mukdahan province. The case has prompted swift action from local officials to contain the outbreak and prevent further infections.

 

 

According to reports shared by the popular Thai social media page “Drama-addict”, the deceased had reportedly come into contact with cattle and consumed beef prior to falling ill. The individual, whose name has not been released, developed a fever and skin lesions and was admitted to Don Tan Hospital on 27 April. He was later transferred to Mukdahan Hospital, where he passed away. Medical tests have confirmed anthrax infection as the cause of death.

 

Anthrax, a serious bacterial disease, can be contracted through skin contact, ingestion, or inhalation of spores, particularly in those who handle livestock or consume contaminated meat. Symptoms include fever, skin ulcers, abdominal pain, nausea, and respiratory distress.

 

In response, Mr. Chakrit Chumchan, the district chief of Don Tan, has issued directives to local authorities, village heads, and related agencies to implement strict disease control measures and monitor for potential further cases. 

 

The public has been warned to avoid consuming meat from animals that have died without a clear cause and to immediately report such deaths to local officials for testing.

 

The Ministry of Public Health is urging anyone who has recently handled cattle, buffaloes, goats, or sheep, or who develops suspicious symptoms, to seek medical attention immediately.

 

Officials continue to monitor the situation closely as containment and investigation efforts proceed.

The Ministry of Public Health has confirmed that 247 people have been exposed to anthrax after consuming contaminated beef distributed during a merit-making event in Mukdahan Province. One fatality has been reported.

 

Dr. Phanumart Yanawetsakul, Director-General of the Department of Disease Control (DDC), stated on 1 May, that the deceased was a 53-year-old male construction worker with a history of diabetes. He developed lesions on his right hand on 24 April and was admitted to hospital on 27 April with symptoms that included darkening of the wound, swelling of lymph nodes in the right armpit, dizziness, and seizures. He later succumbed to the illness.

 

“Doctors suspected anthrax and submitted samples for testing to both the Department of Medical Sciences and Bamrasnaradura Infectious Diseases Institute,” Dr. Phanumart explained. “The results confirmed infection by Bacillus anthracis.”

 

Initial investigations suggest the infection stemmed from the butchering of a cow during a local religious donation event, known as a phapa, with the meat distributed and consumed within the village. A joint team from the DDC, Office of Disease Prevention and Control 10 in Ubon Ratchathani, the provincial public health office, and livestock officials is now conducting an in-depth investigation.

The 247 individuals identified as having had contact with the infected meat include 28 people involved in the slaughter and 219 who consumed raw or undercooked beef. High-risk individuals have been provided with antibiotic treatment, and containment measures are being actively implemented in the area.

Dr. Phanumart warned that anthrax is a serious zoonotic disease caused by Bacillus anthracis, whose spores can survive in harsh environmental conditions for years. The disease commonly affects herbivorous animals such as cattle, buffaloes, goats, and sheep, and is typically transmitted to humans through direct contact with infected animals, raw or undercooked meat, or animal hides and wool.

 

Symptoms usually appear within 1–5 days of exposure and may include fever, nausea, vomiting, severe abdominal pain, characteristic ulcerative skin lesions, and difficulty breathing. Without prompt treatment, severe cases can result in death in up to 80% of cases.

 

Thailand reported anthrax cases in 2000, with 15 infections but no fatalities, 14 in Phichit and one in Phitsanulok. A more recent incident in 2017 saw two cases in Mae Sot, Tak Province, linked to contact with a goat carcass smuggled from Myanmar.

 

Neighbouring countries have also reported cases. In 2024, Laos recorded 129 anthrax cases with one death. Vietnam experienced three outbreaks in May 2023 involving 13 patients and 132 exposed individuals, also linked to beef and buffalo meat consumption.

 

To prevent infection, the DDC advises:

 

1. Avoid contact with cattle, buffaloes, goats, and sheep.

2. Wash hands and body thoroughly after handling animals.

3. Consume only meat certified as safe.

4. Report any unusual animal deaths to veterinary authorities.

5. Seek immediate medical attention if symptoms develop.

New Anthrax Case Confirmed in Mukdahan – Close Friend of Deceased Identified as Infected

Health officials in Mukdahan province have confirmed a second case of anthrax infection following the death of a 53-year-old man who contracted the disease after slaughtering and consuming raw beef. The latest patient, also a 53-year-old man, is a close friend of the deceased and had participated in the same high-risk activities.

 

Dr Narong Chankaeo, Provincial Public Health Doctor for Mukdahan, stated that Don Tan district has now been declared a controlled disease area. All individuals known to have had contact with the infected parties remain within the district. Meanwhile, neighbouring provinces Amnat Charoen and Kalasin have been instructed to heighten disease surveillance.

 

Laboratory testing using RT-PCR methods confirmed the second case on 2 May. The infected man presented with early symptoms, including a lesion on his right hand and rashes. However, as he has no underlying health conditions, his symptoms are currently mild. He is receiving both oral and injectable antibiotics at Mukdahan Hospital and is expected to recover within two weeks, pending monitoring for complications.

 

Two additional individuals are under investigation and awaiting test results, which are expected by 3 May.

 

Dr Narong reported that the total number of people believed to have been exposed to anthrax in connection with the same incident has now reached 638. These individuals include those involved in animal slaughtering or the consumption of raw meat. All identified contacts are being provided with Doxycycline, an antibiotic prescribed at one tablet twice daily for seven days.

 

He stressed that anthrax is a zoonotic disease, meaning it spreads from animals to humans, not from person to person. As such, there is no need for the public to fear human-to-human transmission. Nonetheless, residents are advised to refrain from consuming raw meat during this period and to seek medical attention immediately if any unusual symptoms arise.

The protective suits maybe unnecessary, but easier to be safe than take a risk, however extremely minimal the chances of contamination. I would expect the suits were part of the biosafety protocols issued by the Public Health Department. It also gives reassurance to the people handling the body and coffin.

 

The World Health Organisation (WHO) state “There is no evidence of direct human-to-human transmission. However, spores may contaminate the environment or be released during handling of contaminated remains.”

 

U.S. Centers for Disease Control and Prevention (CDC) state: “Anthrax is not known to spread from one person to another. However, anthrax spores can be present in contaminated materials, including those from deceased individuals.”

 

Anthrax spores can survive on contaminated materials like dead animals and also in the environment for decades, especially in the soil.

In terms of 100% certainty, it’s true that we can’t guarantee absolute assurance based on just a photo. While standards (such as cremation temperature and burner specifications) might appear to be followed, the actual functionality of the burner and the temperature control system in that particular crematorium would need to be tested or verified through more direct means (e.g., equipment inspection, operational logs, temperature monitoring systems, or official certifications).

 

Even if the crematorium looks up to standard in a photo, there could still be underlying issues with how it’s maintained or operated (human factors). It’s reasonable to ask for confirmation from the facility or health authorities, particularly when handling biohazardous material.

 

An instinct to ask tough questions will always be valuable, especially when protecting clients and ensuring everything is in place. And as my wife says, maybe I think a lot, but that's a good thing when it comes to ensuring safety and standards!

Looking at the picture, yes some form of biosafety protocols were followed, hence protective suits, gloves and I would assume masks.

 

A crematorium primarily uses dry heat, using hot air and flames, and normally needs temperatures greater than 760°C, to incinerate a human body.

 

To destroy anthrax spores it is recommended in laboratories, 160°C for at least 2 hours, or 170°C for at least 1 hour, so these temperatures will be exceeded in the cremation process.

 

The greater risk here from anthrax spores, was before cremation, at autopsy, sample taking and testing. I also would assume no embalming took place to reduce exposure risks.

Is Doxycycline available OTC in Thailand?  Seems like a handy thing to keep in the medicine cabinet to get a head start in case it takes a few hours (or days) to get in to see the doc.

Health authorities in Mukdahan province are continuing to monitor a group of 36 individuals who were involved in the slaughtering of a dead cow, following the confirmation of one fatal anthrax case and one further confirmed infection.

 

Dr Narong Chankaew, Chief Public Health Officer of Mukdahan, provided an update on the situation on 3 May, revealing that two additional individuals who had undergone lab testing for anthrax have tested negative. Laboratory results used the RT-PCR method to detect the presence of Bacillus anthracis, the bacterium that causes the disease.

 

The current outbreak emerged after a local man died from anthrax following the slaughter and consumption of raw beef. A close friend of the deceased also tested positive and is currently undergoing treatment.

 

The group under surveillance comprises 36 people who were directly involved in handling or processing the carcass of the infected cow. One additional individual remains under close observation while awaiting test results. Local health volunteers have been assigned to visit those under monitoring at their homes daily.

 

Dr Narong stressed that the situation remains under control, and public health teams are maintaining strict precautions to prevent further spread of the disease.

Health authorities in Mukdahan are scrambling to trace the source of an anthrax outbreak that has already claimed one life and infected two others, as conflicting test results complicate efforts to pinpoint the origin.

 

Dr Narong Chankaeo, Provincial Public Health Officer, confirmed that three cases of anthrax have been identified so far in Don Tan District, one of which resulted in death. Two other patients remain under hospital care, while 98 individuals are being closely monitored for symptoms, primarily those who may have been exposed through skin contact or ingestion. No cases of respiratory anthrax have been detected, which would require a longer 60-day observation period.

 

The investigation has focused on two cattle slaughtering operations. The first, on 12 April, was linked to the deceased and the confirmed patients. Health officials were further alarmed when environmental samples taken from the deceased’s home revealed anthrax spores on both a chopping board and a knife. This discovery suggests indirect contamination and highlights the difficulty in tracing the exact origin of the infection.

 

“We now face the difficult task of determining how the infection entered the first slaughter group, since environmental tests from that location showed no signs of the bacteria,” Dr Narong explained.

 

Questions have arisen about whether the contaminated beef may have originated outside the province. However, investigations have so far yielded no confirmation, and no one has admitted to importing cattle or meat from other areas.

 

In response, local authorities have launched a comprehensive sanitation campaign in the affected area of Lao Mee subdistrict. Disinfectant has been distributed to every household, with particular focus on cleaning food preparation equipment. Talks are also underway about providing new chopping boards and knives to residents to restore public confidence.

 

When asked about the effectiveness of the current decontamination measures, Dr Narong said follow-up tests might be necessary, though this decision lies with the provincial livestock department.

Health authorities have confirmed a fourth case of anthrax infection in Don Tan district, Mukdahan province, raising concerns about possible environmental and meat-related sources of the deadly bacterial disease.

 

Dr Narong Chankaeo, Provincial Public Health Officer for Mukdahan, announced on the 6 May, that the latest patient is a woman and the wife of one of the previously confirmed cases. All four cases identified so far are believed to have been exposed through small cuts on their hands, allowing the anthrax bacteria to enter the body. In the latest case, the woman sustained a knife wound to her hand while preparing beef for cooking.

 

The new case had already been classified as high-risk and was under medical observation as of 5 May. Following this diagnosis, there are currently no more patients awaiting test results.

 

Dr Narong added that 538 individuals who had close contact with the infected patients have now completed their monitoring period, leaving just 98 people still under observation. These individuals are expected to clear the 7-day surveillance threshold by 7 May. However, the Ministry of Public Health has mandated continued self-monitoring for 60 days in total, with instructions to report any unusual symptoms to local health authorities.

 

Authorities have distributed a QR code via the Line messaging app to help people self-assess and report symptoms. The symptoms to watch for include black skin lesions on the hands, nausea, vomiting, diarrhoea, difficulty breathing, and persistent cough. According to epidemiologists at the Department of Disease Control, there is currently no evidence of respiratory transmission leading to pulmonary anthrax.

 

The suspected sources of the anthrax outbreak are under investigation, with two primary locations being scrutinised. The first is a local slaughter site in Ban Khok Sawang village, where cows were reportedly butchered directly on the ground, without sanitary containment. Officials suspect the soil, commonly used in rubber plantations, may have been contaminated with anthrax spores.

 

A second possible source is an area along the Mekong River, where flooding in the past may have deposited bacteria carried by animal carcasses. On 5 May, disease control teams collected soil samples from both suspected sites. Results are expected within four days.

 

Neighbouring provinces, including Amnat Charoen and Kalasin, have also been alerted due to the potential spread of infected meat during communal festivities.

 

Health officials continue to stress vigilance and public cooperation as investigations proceed. If confirmed, this would be one of the few recent anthrax outbreaks in Thailand linked to environmental and meat-handling practices.
