30 day extension for 60 day visa-exempt.

Morning.
I'm wanting to get 30 days extension in C Mai, on a 60 day visa-exmpt entry. I'm guessing this is reasonably straight forward. What are the basics I need?
Do I need to have a hard copy of my TDAC QR, or is it ok tp have it on my phone only?
Do I need to show a ticket out? (I have a ticket out but not for another 3 months).
Is there any kind of questioning/interview?
This will be my first time extending.

You do not need proof of onward travel and you do not need a tdac number.

You do need a TM 30 receipt showing your registered as staying in a place served by he Chiang Mai immigration office.

You can get your 30-day extension at the main immigration office by the airport or at the satellite office at Central festival Mall

You will need to filled out tm7 application form, one passport size photo, copies of the data page of your passport, your most recent entry stamp and 1900 baht

In Chiang Mai you can apply for an extension when you have 45 days or less left on your current stamp there is no need to burn your stamp down to the last day because they add the 30 day extension to your current expiration date, meaning you don't lose any time applying early.

Are these extensions still limited to one per calendar year, or has this restriction been dropped?

I would make travel plans based on one extension.

Keep in mind that at some point visa exempt will be 30 days. They may remove the one extension then.

Obtaining a SETV is also an option

With respect to the 1/year, we do plan to come back on a SETV, and ask nicely for a 30 day extension on that. (At which point we would be flying home on booked tickets). Is that going to be considered a "second extension" or, would it be treated differently as we will be holding SETV (as opposed to a 60-day visa exempt entry)?

Obtaining the SETV is a good idea and you will be able to extend that by 30 days.

The "second extension" relates to visa exempt entries

No issues on getting first 30 days. Forms, queue, wait, pay, queue again, go for lunch, come back, wait, stamped and gone.
