Have downloaded the above and note that in segment (g) My Permanent address is ... It states '(preferably in country of nationality)'. Bearing in mind I've lived here in excess of ten years and have a company and a house here, should that not be my permanent address? Or will it be construed by Thai officials that I am taking liberties in assuming I have a 'right' to a permanent address here? It'd be a bit odd giving the address of a house in the UK I sold years ago. Surely they don't think everyone who marries here has just stepped off the boat?

Also, I had my passport renewed at the UK embassy in Bangkok, so what do they expect me to put in 'issuing office', when all I can see are the letters GBR under a 'code of issuing authority' in little red type at the top of the page?

Any advice appreciated. Don't want to waste any more time than is strictly necessary having to have the thing retyped at some local shop. Thanks in advance.

I used my last address in the UK although like yourself, I moved on years ago. Alternately, you can use a relatives address. Nobody is going to check.

For your passport information, say "British Embassy, Bangkok". There will be no problem as that is where it was issued.
