Passport and copy.

Marriage Certificate and copy.

Wifes ID Card and copy.

Letter from Wife stating that she wants you to stay with her.

Bank Book. Balance amount ????. Some reports say a little , some say a lot.

The requirement of 40k/month is for an annual extension of stay from an immigration office, not for a non-O visa from a consulate.

the bank book, does it have to be a thai bank account or do offshore bank accounts work too?

For an application for a visa at a Thai consulate, the money can be in any bank anywhere in the world.
