The quickest way I know of is Khota Baru in Malaysia it takes only an hour or so to get the actual visa done, You have to have your paperwork that is needed for the type of non immigrant visa to which you are applying for, The people arranging the visa trips have good connections at the Consulate and can sort out small problems with missing paper work, If you have a few things missing it will be ok but you need to have at least some type of proof why you require a non immigrant visa,

You can arrange to fly to Narathiwat and take a mini van to the border town of Sungai Golok and drop of your bag if needed ,check in to your hotel, You then cross over the border just near the hotel and the mini van drives you to 1 hour to Khota Baru and you wait at McDonalds for an hour while they take your passport to the Embassy and get your visa, You then drive back to Sungai Golok Thailand for the night and the next morning get the van back to the airport at Narathiwat,

The pluses for this trip is that you are in Malaysia only about 3 hours, you can stay in Thailand the night at Sungai Golok, If you leave from Bangkok the whole trip takes just over 24 hours, Leave Bangkok at 7.00am and back at 12 lunch time the next day, The minuses is you spend 3 hours in a mini van the first day, the cost is around 1,100 baht for the visa, 1,000 baht for the mini van and visa service, airport transfers approximately 200 baht total for both ways, the hotels are cheap here and good ones are under 1,000 baht, The airfare round trip from Bangkok is approximately 7,000 baht with Air Andaman. Sungai Golok is border brothel town by the way and is a very sleazy place.

A look in Bangkok Post, Pattaya Mail and Phuket Gazette etc.(paper editions) shows several companies advertising group tours to the borders, some of them to Thai Consulate in Malaysia.

You will propably find agents in Kota Bahru, just ask at any hotel for the "Visa man".

As you will propably get only single or double entry in Malaysia, it's enough if the passport is valid for 6-12 months.

18 months validity is usually required for multiple one year visas.

The visa runs are done only on Tuesdays now, you can get a double entry Non B Visa, you can not get a one year multi entry visa ,A good cheap hotel to stay in at Sungai Golok is called Grand Gardens and is only 600 baht a night for the cheapest room, the number to book is 073 613600, The visa arrangements can be made with a girl called Meow her number is 073 611417, The visa cost is 110 ringits and the visa girl Meow charges 1,000 baht for the mini van and doing the visa, Check with Meow what paper work you do need as she can fix it if you have most of the paper work and some missing paper work should be ok, she does have a good connection there so its worth to check with her what you need first so not to waste your time getting loads of stuff together you do not need, if you call her she will arrange for you to be picked up from the airport and handle the whole trip for you, If you do it from Phuket due to the flights schedule you need to stay two nights but from Bangkok you only need to stay one night,
