If someone has entered on a Tourist Visa for 60 days, extended it for another 30 days, and

then did 2 border runs for the free stamps, can he do another "run" or will he have to do

a Visa at a consulate?

I think he will have to go to get a TR visa as the extension is a stay not on a "visa".

No it does not count. It is an extension of a visa not a visa exempt entry.

One more run should be okay.

do NOT count against the 90 days in 6 months of Visa Exemption,

so you are OK for one more run.
