You need to contact the Land and Transport Department. They issue the International Driving License. In Bangkok is it near Chatuchak on Pahonyothin Road.

Yup ^^^. Your lady will need a 'full' i.e. 5 year Thai licence.

Not all offices issue international permits so you'll need to ask at your local office (if in BKK Mo-Chit definately do issue).
